Please do not submit confidential health information through website contact forms unless directed to do so through secure channels.

SMS communication consent
By providing your phone number and consenting to treatment, you agree to receive SMS messages from Grounded Therapy related to your care, including appointment reminders and care coordination. Message frequency varies (typically 1–4 messages per week). Message and data rates may apply. You can reply STOP to opt out at any time or HELP for assistance. SMS consent is not shared with third parties. We do not sell personal information.
Data security
We implement reasonable administrative, technical, and physical safeguards to protect website data. However, no method of internet transmission is 100% secure. Submission of information through online forms is at your own discretion.
Data retention
Website inquiry data is retained only as long as reasonably necessary for administrative and legal purposes. Clinical records are retained in accordance with applicable federal and state laws and our HIPAA Notice of Privacy Practices.
